REIMBURSEMENT FORM

Thank you  for your help with our PTO events. We want to get you reimbursed as quickly as possible so please follow the steps below:

1. Complete the Reimbursement Form found here: PTO REIMBURSEMENT FORM

2. Collect your receipts for all items being reimbursed.

3. Drop both the form and the receipts in the PTO mailbox inside the main office doors at Johnson Elementary OR email scans/images of both to Shanna Novosel at snovosel@biggby.com
